The Reservation Form is an Adobe Acrobat file. It is designed to be filled out online. Please read the instructions below:
If you are a Microsoft Internet Explorer user, I suggest saving the form before attempting to fill it in. Then open the file with Acrobat Reader. Internet Explorer doesn't like the form. If you have Firefox, Mozilla, Safari or Google Chrome, then you shouldn't have any problems.
Once you have filled in the Classmate Info and Guest Info fields, click the Main Dish Selection checkboxes for you and your guest, to identify your main dish selections.
Then check the appropriate checkboxes to show which activities you will be attending and input the number of guests who will be with you at the Elm Grove luncheon. I've included a cost calculator, so after you make your reservation choices, click the "Cost Calculator" button and the form will show your total reservation cost. If you want to "what if" with the calculator, just click the button after making your changes.
Finally, print the form, fill out a check or money order for the total amount and send the form and payment to the address shown on the form.
NOTE: You will not be able to save the finished form, so I suggest printing two copies, one for your records and one to send in with your payment.
